Service Options Using Resources in a Community Environment (SOURCE)
The SOURCE program is designed to assist individuals with chronic health conditions who have limited financial resources and cannot afford the cost of nursing home care. The goal of SOURCE is to improve the well-being of individuals by providing nursing and personal care services in their homes or communities, helping them avoid placement in hospitals or nursing facilities.
Through the SOURCE program, eligible individuals receive care from skilled nurses and healthcare professionals in their own homes. This not only supports their recovery but also helps them maintain a life as close to normal as possible, surrounded by their families and communities.
Dunn-Harris Healthcare Services is an approved home health care provider through the State of Georgia’s SOURCE program. If you or a loved one qualifies for the SOURCE waiver, our services may be provided at little to no cost to you, as we are reimbursed directly by the State of Georgia.
Am I Eligible for Georgia SOURCE?
To qualify for the SOURCE program, you or your loved one must meet specific eligibility criteria:
- Age Requirement: You must be 65 years old, older or under 65 with a severe disability.
- Chronic Medical Condition: Applicants must have a chronic health condition that requires ongoing care.
- Medicaid Coverage: You must have full Medicaid coverage and meet all other eligibility requirements for the program.

- Respiratory and pulmonary issues
- Diabetes
- Arthritis
- Heart disease
- Amputations
- Alzheimer’s disease
- Spinal cord injuries
