Skilled Nursing

The Registered Nurse (RN) demonstrates clinical expertise in nursing assessment and nursing diagnosis and advanced proficiency in technical skills necessary for developing, providing, and coordinating care to patients.

Education:

Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree.

Licensure/Certifications:

Must have current licensure by the Georgia State Board of Nursing and valid CPR certification. Must possess a current valid Driver’s license. Must have “Active” license, “No Restrictions”.

Previous Experience:

Previous experience in Med/Surg, Critical Care, Oncology or ER experience. Preference given to those with 2 or more years of recent experience in one of these areas. Previous Home Health experience is preferred but not required. Clinician must have demonstrated strong clinical skills and critical thinking.

Knowledge/Training:

Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health care services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process and collaborating with interdisciplinary team.

Computer Skills:

Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Transportation:

Must have valid automobile insurance and reliable transportation.

Assessment/Administration:

Tube Feeding.

Home-Health Aid/Certified Nursing Assistant

The Home Health Aide (HHA) Primary’s function is to provide personal care to the client in the home; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with manager.

Education & Qualifications:

Education:

High school graduate, GED or equivalent in training and experience. In addition, must have successfully completed the HHA Certification program.

Licensure/Certifications:

Certification as Nursing Assistant

Previous Experience:

At least 1 year experience as a Home Health Aide in a hospital, nursing home or home health agency, or within the last 3 months has completed HHA program.

Knowledge/Training:

Demonstrate interest in the welfare of the ill, elderly, adolescent, young children and desire to provide care in the home.

Skills:

Must be able to read and write in English and follow written and verbal instructions in English effectively.

Transportation/Escort Services:

Must have reliable transportation

Companion Care:

Monitor Physical and Mental Condition

Light Housekeep & Laundry Services:

Meal Preparation:

Advise Family on the proper ways to lift, turn and position patients

Personal Care Worker

Education:

High school graduate, GED or equivalent in training and experience.

Licensure/Certifications:

None

Previous Experience:

At least 1 year experience within the home health industry

Knowledge/Training:

Demonstrate interest in the welfare of the ill, elderly, adolescent, young children and desire to provide care in the home.

Skills:

Attention to detail; interpersonal skills; stamina and time management. Must be able to read and write in English and follow written and verbal instructions in English

Transportation:

Must have reliable transportation
