- Be informed about the plan of services and be involved in the development plan.
- Be informed promptly about any changes in services before the change.
- Accept or refuse services.
- Be informed of the charges for the services provided.
- Be informed of the contact number(s) of the supervisory personnel.
- Be informed of complaint procedure.
- Have confidentiality of client information.
- Have property and residence treated with respect.
- Written notice of the contact information of the state licensing authority.
- Right to a copy of the PCHP’s most recent report from a licensure inspection.
- Responsibility of the client and/or responsible party to inform the provider of any changes in the client’s condition.
